From a skim of wikipedia, it looks like most fish "breath" in through the mouth and out through the gills, so the cigarette would probably be in the mouth
im pretty sure the water goes into the mouth and out of the guills, so a cigarette on the guills would be like putting it in your nose while breathing in only through the mouth and breathing out only through the nose
I've never actually smoked one so I'm definitely not the best source! I know what its like breathing it secondhand - I can't imagine what it would smell like if you smoked it through your nose haha
