The right side of his face (left side in the portrait from our perspective) looks a little droopy like he had a stroke.

Dumbass wants to come off like big brother in 1984 adorning the walls of government buildings with his mean-mugging and obviously intentionally ominous face. He wants to be a tough guy villainous dictator like Putin and Kim. He thinks this makes him look scary.
