Depending on the ending you picked for the game you could argue he's more alive by the end of the game than the start. It wouldn't be too much of a stretch to label that particular ending the canonical one and co tinue from there. Besides, Arasaka is still around, just beheaded. I can totally see a plot where Johnny continues his revenge mission against them.

There is absolutely no reason a Johnny Ingram couldn't be put in a Dragoon cyborg body or something similar. Shoot in Red there are even cyborg bodies that look completely human.
