So if I’ve selected two languages in my settings, if I see language outside those selections can I assume that is because their language settings aren’t set properly or have I stuffed up a setting?

When making a post in Lemmy, specifying a language is optional, which causes many many posts to have useless language metadata.
They're not that keen on fixing it, either - https://github.com/LemmyNet/lemmy-ui/issues/2317
