There's a pretty consistent to pull off route through Quarry Junction that will put you on the Camp McCarran monorail within 5 minutes of leaving Doc Mitchell's house. Imo it's a lot more fun to start the game by going to the Strip directly than the slow walking simulator that is the intended route
