Writers of serial fiction: how do you stop continuity becoming a second job?

Do you use a wiki, notes app, spreadsheet, character sheets, or something else? What is the first continuity problem that tells you your system has broken?

To the truth, whether it's scientific truth or historical truth or personal truth!

And if you can't find it within yourself to stand up and tell the truth about what happened, you don't deserve to wield that pen.
