This episode put so many pieces in the board I don't know what is going on. Macon knows about aliens and has alien detectors. The sheriff knows all this. At least one Manhunter is in the town. How much does Hal know about Manhunters? Why doesn't he trust the Guardians? Why did he go directly to Sinestro? Why is the hospital basically empty? Is everyone an alien? Why does Hal have a carnivorous plant? Who exactly are the dudes that half kidnapped John? Is the bird really just a bird???
Also loved that Hal saying the Green Lantern oath in such a half-assed way. I wonder if we'll get it every episode in an unsatisfactory way... And then we'll get it perfect.
It's a great question. The simplest answer is prison chatter. Even though Sinestro is behind an extra security door it wouldn't be surprising that he had some occasional contact with a prison guard. It's also possible a Guardian or someone more important occasionally comes to him for information similar to what Hal is doing.
The other option is that John is a long term plan by Sinestro. Look at John's father and upbringing. John's dad is raising him to not be afraid, the "perfect" Green Lantern. But how does John's dad know that is the training? The interview is the first time Hal is officially going public. It's clear John's dad has been shooting at John for a while.
John, as a replacement Green Lantern, is Hal's biggest fear. Sinestro has just been waiting for Hal to show up. He isn't surprised to see Hal. He, directly or indirectly, positioned John Stewart so he knows who he is.
She's definitely an "outsider", but I don't think she's a Manhunter. We learned this episode that she's from Gotham and she talks about having to earn the respect of the town; specifically from her father in law which is also interesting.
But she's absolutely hiding something. Whenever Hal reveals something, she already knew about it. John points out the alien in the first episode, Hal agrees but says he isn't telling Kerry, so John tells Kerry but she already knows. This episode Kerry already knows about the alien detectors.
But Kerry also seems to be covering up at the big town meeting. She seems to explain away the weird hospital with no other people and fresh equipment.
So she knows something. She is fairly straightforward with Hal in some cases, so she's hiding something, but I don't think she's a Manhunter... Unless there are multiple Manhunters.

Also why does Sinestro know about John Stewart?

My hot take is that Kerry is a reformed Manhunter, and that is why the guardians let her go live on Earth
