Seems like these just say the vaccines are approved for people under 65 with an underlying condition and everyone 65 and older. I don’t think that has changed since last year, but CVS was not asking about underlying conditions when I got it back then (in case anyone without an underlying condition in that age range wants to get it this year).
According to the CDC, underlying conditionsplacing people at risk for higher risk of severe COVID-19 include but are not limited to cancer, asthma, chronic lung disease, diabetes, heart conditions, mental health conditions and pregnancy.

Even if they do ask they don't have your medical records to check against. Just tell them you have one of them.
