don't know details about treatments, not sure is it because some hormones and substances are forbidden in sports anyway (?)

maybe the best solution is to have 3rd (or even more if needed) category in sports, male, female, trans-persons?
another polarity would be to put everyone in one category
every one can choose who to compete against based their preferences, not sure if level field can be reached as each body react different to substances
