That field is adjusted based on your response on the first page. If you responded to "How satisfied are you with Firefox?" with "Dissatisfied", the freeform asks why you are dissatisfied.
Some laws in some regions define adults as 19, not 18. I guess they just blanketed everyone under one age limit (I'm assuming [so they could] to hide your location from the survey).

Other than the double negative on that one question, I don't really see what makes this survey "crappy"? Pretty standard stuff.

They had a freeform for why we're satisfied, but nothing about why we're dissatisfied. A bit disappointing.

These are too general and subjective questions and I think deeper surveys are needed.
