Does a heavy slam not count as a "heavy attack"? when i put the Killing Blow mod on, the heavy attack stat increases but not the slam damage stat underneath it.

No, the slam is a different attack. It also does not increase the damage of thrown glaives.

It increases the damage of the explosion which is the main part of the damage anyway but the projectile itself is not affected
