I had insurance from my job, and also insurance on my husband's plan because he forgot to cancel it in time. I put in a claim for a leg X-ray after falling. They refused the claim, and only after reading the small print did I realize it was because they had the wrong primary insurance on file. I didn't know that was a thing, just figured two = double plans. The letter they sent seemed to actively avoid explaining why I was refused.

I've long said that for anyone guilty of a capital crime, death was far, far too merciful. Honestly we need to start trying these rich healthcare exec bastards for manslaughter.
