That was the last name of his adoptive parents. Klingons do not use last names. I don't think Worf ever used that name himself. I am unsure why Alexander used that last name, though. Perhaps as a way to rebel against his Klingon heritage.

At some point he says "son of Rozhenko, House of Martok " yadda yadda. But you're right, the last name is really only used as a title/formality when needed.

One or another of the 1990s TNG video games let the player look up random info in the ship's computer, and the personnel file for Worf gave his full name as Worf Rozhenko.

No, it's Woof. He didn't like using that name because he thought it brought shame and dishonor to his family, so he opted for "Worf, son of Mogh". Lwaxana knew what she was doing when she called him Mr. Woof.

Yeah. Worf Worf and his brother Alexander. Gold Worf and Silver Worf. No one remembers Alexander. Worf Worf even named his first born after his brother. No one remembers him either. All live in Worf Worf's shadow.
