"1st, 2nd, and 3rd World" designations were always problematic, but at least they made some sense in the 20th century Cold War.
The "1st World" is Western Democratic Capitalist States. (The US and NATO nations).
The "2nd World" was a designation for Soviet states. (Though, if we were to modernize this language, I would say that BRICS is our contemporary "2nd World".)
The "3rd World" was just "everyone else", but eventually became synonymous with "poor and industrializing nations".
Made another comment, but I'll also respond here, with a real explanation. 'The Fourth World' consists of indigenous peoples. Typically those who's integration into institutions of modernity are in some way tenuous.
So, think Bedouin tribespeople with smartphones, or nomadic pastoral peoples in the Sahel who regularly cross national borders to graze their cattle, or indigenous Americans on reservations who's sovereignty is always contested.

If the First world is the US and its allies, the Second world is the USSR and it's allies, and the Third world is all of the non-aligned countries, then Fourth World theories assert that the Fourth World consists of indigenous peoples who's integration into the institutions of modernity are tenuous.
