It took quite a lot of evolution and mutations for a scorpion to grow to that size. One such mutation reversed the direction of the stinger. I expect this answered everything, no further questions.
I just watched Possession for the first time this past weekend and it was the first thing that came to mind when I scrolled past this image. Sam Neill was so young!
