Growing up in Canada, McDonald's was provided to kids in hospital through the Ronald McDonald House Charities! I have fond memories of getting bloodwork done and being treated to McDonald's because my parents never took us there...
I imagine most people that maintain an extreme level of "junk food is cancer" attitude are doing it for medical reasons or social clout. Not because they didn't like it.
When I came back to school, I’d keep filling that McDonald’s cup at the water fountain to get maximum mileage out of it. Keep flexing that bad boy until the bottom soaked out.

I think they mean that they got treated to happy meals after a doctor's visit and teased their fellow schoolchildren with that fact?

There was a bakery next door to my childhood doctor, so it was giant cookie time after getting checked out and sometimes stuck with needles.
