I think it's a conscious refusal by them to buy over a certain size. I get it, I dealt with it when I had to move to XL. Both he and his brother are constantly OBSESSED with their physical images, and it wouldn't surprise me if the reason they get things sized WAY too tight and suchlike is pure body dysmorphia.
