There's actually a volume toggle so he can be quiet when he needs to be or extra loud when he wants to be more intimidating. Anakin/Vader has always been drawn to dramatic.

That is exactly what he's doing. When he jumps Luke in ESB, he's breathing harder and faster for a bit once the breathing sounds come back on.

I’d imagine if a nacho got stuck in the breathing machine it could make a high pitched “WheeEEEEeee!” Which would have disturbed the audience.
